A Patient’s First Visit

Your initial appointment will consist of a consultation with Dr. Willden that allows you the opportunity to discuss your dental concerns and oral health goals. Afterwards, we will gather the necessary diagnostic information to help formulate a treatment plan. Dr. Willden will then spend time with you, one on one, explaining your diagnosis and treatment options. Occasionally, treatment can be performed the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

• A completed Medical/Dental History Form
• Any recent dental X-rays (if applicable)
• A list of your current medical prescriptions and any other vitamins, supplements, etc.
• If you have dental benefits, please bring your card and any associated documents. This will save time and allow us to help you process any claims.

Be sure to alert the office if you have a medical condition that may be of concern or if you are on any medication.
